Position: Project Manager, IT -1099 Contract To Hire

Monogram Health supports patients suffering from Chronic Kidney Disease and End Stage Renal Disease by forming deep rooted relationships and preparing them both emotionally and physically for the challenges of managing kidney disease. Monogram Health uses next generation artificial intelligence algorithms to predict necessary and timely care to promote the delay of kidney disease progression, seamless transitions to dialysis and/or pre-emptive kidney transplant. We then utilize in-home visits to build meaningful relationships between patients and their care team that drive positive behavior change and optimize our patients’ health outcomes.


The Project Manager is accountable for implementation of new clients, as well as oversight of operations to support current clients. Accountable to drive a successful project management approach, ensuring repeatable positive outcomes. The ideal candidate is detail oriented, a strong team player, and has excellent prioritization and time management skills. Primary responsibilities include 1) Managing contractual requirements to ensure deliverables and reporting are timely and accurate 2) Establishing and managing project implementation plans for new client implementations 3) Collaborate across the organization, with various stakeholders to ensure all deliverables and milestones are met to exceed contractual and client expectations.


Roles and Responsibilities

  • Own and manage technology project lifecycles from planning to execution, ensuring on-time and within-scope delivery.
  • Develop detailed project plans, timelines, and milestone trackers using Smartsheet, Azure DevOps (ADO), or similar tools.
  • Collaborate with technical leads to translate business and clinical requirements into technical tasks and deliverables.
  • Coordinate across cross-functional and technical departments to align priorities and schedules.
  • Monitor system integrations, data workflows, and other technical dependencies to prevent delays and ensure quality outcomes.
  • Actively identify risks, issues, and roadblocks—drive resolutions by working with technical teams and stakeholders.
  • Facilitate regular project status updates, stand-ups, and reporting for internal and external stakeholders.
  • Act as a flexible and supportive leader, stepping in to assist team members as needed.
  • Review and monitor project deliverables and reporting to stakeholders to ensure timely delivery and highlight any risks, issues, dependencies and decisions.
  • Delivering successful outcomes consistently
  • Communicate project updates clearly to both internal and external stakeholders.
  • Promote a positive work culture that supports employee growth and demonstrates empathy toward patients.
  • Support Monogram’s mission to provide personalized patient care by fulfilling assigned responsibilities.
  • Perform other duties as assigned.


Position Requirements

  • Bachelor’s degree from a four-year college
  • Experience leading projects involving healthcare data integration, platform implementations, or system migrations.
  • Exposure to Agile or hybrid delivery methodologies.
  • Project management certification or 5+ years’ experience with program management, project management, or similar experience
  • 5+ years’ experience working for a value-based provider group, health plan, and/or vendor contracting with health plans delegated for services OR working in healthcare
  • Detail-oriented, excellent prioritization and time management skills
  • Expertise in partnering with internal and external IT departments is highly preferred
  • Proficient in MS Suite, Smartsheets and ADO.
  • Experience working in high-growth environments preferred

  • Valid driver’s license
