About the role:
Accompany Health Physicians serve a key role in our Integrated Care Teams, which also include Advanced Practice Clinicians, Community Health Workers, Patient Experience Navigators, RNs, Social Workers, Behavioral Health Clinicians, Psychiatrists, and Pharmacists. Together this team is responsible for providing and coordinating holistic, patient-centered care for an intimate panel of patients with complex medical, behavioral health, and social needs.
As an Accompany Health physician, you will co-manage and provide clinical supervision for a longitudinal panel of patients with 4-6 advanced practice clinicians. You may also independently carry a small panel of high-risk, complex patients. You will remove barriers to accessing care and bring care to our patients- seeing them in the home, community, and virtually. In collaboration with our APCs, you will ensure comprehensive holistic care, chronic disease management, excellent preventative care and wellness, and prompt urgent care as needed. Our smaller panel sizes paired with robust support from a multi-disciplinary team means that you will get to spend more time with their patients and have an even greater impact on their health and well-being.
We expect ~50% of your time will be spent providing direct clinical care ~1 full day and 1-2 additional half-days in the field with an additional ~1 half-day of care provided via telehealth. The rest of your time will be spent in a clinical supervisory and management capacity (e.g. case conferences, external provider communication, etc).
Clinical responsibilities will include:
- Directly supervising a group of 4-6 Advanced Practice Clinicians, including overseeing co-management of APC panels to ensure exceptional, evidence-based, and patient-centered primary care for a longitudinal panel of patients.
- Serving as the primary clinician in a small physician-led panel of high risk, complex patients.
- Performing home and community-based follow-up and urgent care visits as well as virtual visits in the care of your co-managed patients.
- When needed, performing annual wellness visits and patient assessments to obtain a comprehensive biopsychosocial view of patients’ health with a focus on thoughtful, accurate, and specific documentation and addressing quality gaps in care.
- Facilitating case conferences in partnership with Care Team Managers (CTMs). This includes guiding the teams to create appropriate patient-centered care plan that address root drivers of health and collaborating with CTMs to ensure execution of the care plan by all members of the multidisciplinary team.
- Collaborating with external providers and community-based organizations to advocate for patient care aligned with their goals.
- Providing clinical coverage as needed, including performing home-based follow-up and urgent visits when appropriate.
- Participating in a weekend and off-hours on-call schedule to address patients’ urgent clinical needs.
- Ensuring timely and appropriate documentation within our electronic medical record as well as our custom-built platforms that support care for our patients.
Managerial responsibilities will include:
- Ensuring timely, accurate, and appropriate documentation for all APC visits under your supervision.
- Ensuring quality gap closure for your co-managed panel of patients.
- Performing 1:1 in-person observations as well as case and chart reviews on a regular basis.
- Coaching and training advanced practice clinicians in clinical care, workflows, EHR documentation, and multidisciplinary team collaboration.
- Participating in workflow, policies & procedures improvement and implementation.
- Participating in interviewing and hiring advanced practice clinicians and fellow physicians.
- Being a “culture” carrier and leader for the market.

Desired skills and experience:
- Required
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate
- Active Internal Medicine or Family Medicine Board Certification
- Active Medical license in the state in which you are seeking employment
- Active DEA Controlled Substance Registration. X-waiver licensure (for buprenorphine prescribing) is a plus but not required
- Active controlled substance license in state in which you are seeking employment or eligible for application
- Certified in Basic Life Support for Healthcare Providers
- 3+ years of clinical experience in providing primary care in an internal medicine, family medicine, or geriatric medicine setting
- Comfort and experience in working within an electronic medical record
- Valid unrestricted driver's license and access to an insured vehicle for daily use
- Preferred
- Fellowship training in Geriatrics, Addiction Medicine, Palliative Care, or other professional degrees (e.g., M.B.A., J.D., M.P.H.), are welcome but not required
- Experience and passion for caring for individuals with complex chronic medical, behavioral and social needs in a multidisciplinary model
- Experience with co-managing patients with Advanced Practice Clinicians
- Experience within a value-based care model
- Experience providing home-based care
- Experience and comfort with engaging individuals with untreated and/or symptomatic chronic mental illness and addiction
