About the role:
Accompany Health Medical Directors serve as a key leader in our markets for our Integrated Care Teams. Our teams include Advanced Practice Clinicians, Community Health Workers, Patient Experience Navigators, RNs, Social Workers, Behavioral Health Clinicians, Psychiatrists, and Pharmacists. Together this team is responsible for providing and coordinating holistic, patient-centered care for an intimate panel of patients with complex medical, behavioral health, and social needs.
As an Accompany Health Medical Director, you will co-manage and provide clinical supervision for a longitudinal panel of patients with 2-4 advanced practice clinicians. Depending on the size of the market, you may also supervise Associate Medical Directors who will each oversee up to 6 APCs on their own. You may also independently carry a small panel of high-risk, complex patients. You will remove barriers to accessing care and bring care to our patients - seeing them in the home, community, and virtually. In collaboration with our APCs, you will ensure comprehensive holistic care, chronic disease management, excellent preventative care and wellness, and prompt urgent care as needed. Our smaller panel sizes paired with robust support from a multi-disciplinary team means that you will get to spend more time with their patients and have an even greater impact on their health and well-being.
We expect at least 40% of your time will be spent providing direct clinical care ~1 full day, ~1 additional half-days in the field and an additional ~1 half-day of care provided via telehealth. The rest of your time will be spent in a clinical supervisory and management capacity (e.g. case conferences, external provider communication, etc).
Responsibilities will include:
- As a Medical Director, you will:
- Lead teams of advanced practice clinicians (APCs), Physicians, Community Health Workers (CHWs), Patient Experience Navigators (PENs), RNs, behavioral health care managers (BHCs), social workers, pharmacists, and pharmacy technicians in an integrated care model
- Directly manage and oversee advanced practice clinician team managers and physicians within your market. This includes providing clinical mentorship, oversight and supervision, as well as coaching and performance management of the team to ensure appropriate and accurate clinical diagnosis and documentation, quality gap closure, and evidence-based clinical care to prevent avoidable medical expense
- Partner with operational and nursing leadership in the market to ensure appropriate daytime and off-hours clinical coverage for inbound triage and urgent needs
- In partnership with operational leadership, drive strategic implementation and integration of clinical innovations within your market that drive improved patient quality of care, total cost of care, appropriate risk adjustment, and efficiency
- Monitor, evaluate and implement quality improvement activities, including review of patient and care team quality of care and safety concerns
- Lead the hiring, onboarding and continuing training of clinical team members within your market to ensure their success
- Participate in the development and delivery of educational content for clinical care team members
- Assist in developing relationships and appropriate collaborative workflows with network providers and community partners in the market
- As an Accompany Health Clinician, you will:
- Aim to spend at least 40% of your time on Direct Patient Care (in-home and virtual). This may be adjusted based on patient need in your market
- Co-manage a panel of complex members with advanced practice clinicians in a value-based care setting to provide diagnosis, treatment, counseling, medication management, acute triage and follow-up care
- Establish and maintain trusting relationships with patients and their caregivers to assist in achieving their goals
- Communicate with network PCPs, specialists, provider organizations and community based organizations to facilitate collaboration on excellent patient care
- Work daily within our EHR and custom-built technology platforms
- Maintain professional credentialing and CME standards
- Participate in a rotating on-call schedule that will take place outside of normal business hours as urgent patient needs arise

Desired skills and experience:
- Required
- M.D., D.O., N.P., or P.A graduate
- Board certification in Internal Medicine, Family Medicine or Medicine-Pediatrics required for M.D. or D.O., Board certification in Family, Adult, or Gerontology for A.P.N.
- Fellowship training in Geriatrics, Addiction Medicine, or other professional degrees (e.g., M.B.A., J.D., M.P.H.), are welcome but not required.
- Experience building and leading multi-disciplinary teams, supervising clinical team members, and have held leadership and/or administrative roles in clinical organizations or other innovative care models
- Experience with change management and process improvement theory, techniques and practical application
- Experience using a metrics-driven approach to analyze cost, quality, and satisfaction data to drive clinical strategy and program redesign
- Comfort with working in an electronic medical record
- Unrestricted driver’s license and vehicle for daily use
- Preferred
- 5+ years of experience in an outpatient primary care practice
- 2+ years of experience practicing in a value-based care model
- 2+ years of experience in direct management of APCs or physicians
