Job Description
Ideal candidate should be remotely based near Metro Denver (within 1-2 hour radius) and be willing to commute into office when needed. This is a 18-month contract with the State of Colorado.
Job functions
- Work remote and at clinic site as health coach for client’s employees with chronic conditions as part of a clinically integrated, long term health improvement program
- Use specific motivational interviewing techniques and strategies to resolve ambivalence and support self-efficacy
- Conduct candidate screenings for various health risks
- Conduct assessments of health status, health engagement and health risks
- Develop person-centered treatment plans, provide education materials and ensure quality, effective care recommendations
- Evaluate effectiveness of care plan and progress made by participant
- Engage in skill-building, goal-setting, resiliency-building, and healthy living skills
- Coordinate care with the clinical care team at clinic and EAP
- Chart interventions and progress in case management system and Excel spreadsheets
- Collaborate with participants and physicians to formulate an optimal plan of care that meets all of the medical, psychosocial and financial needs of the participant
Minimum Qualifications:
- Nationally Board Certified Health and Wellness Coach (NBC-HWC) or Certified Health Education Specialist (CHES) or Master Certified Health Education Specialist (MCHES)
- 2+ years clinical or community health experience required
- Motivational Interviewing training and experience required
- Bachelor’s or Master’s degree in health science, psychology, behavioral sciences, public health, counseling, social work, or nursing preferred
- Candidate should have strong experience working with people to improve their health
- Candidate should be health-centered and should be able to demonstrate healthy lifestyle skills
- Demonstrates integrity, initiative, good judgment and teamwork
- Strong interpersonal relationship & communication skills, and the ability to work autonomously with a diverse patient population
- Intermediate level of experience with Microsoft Office products, with the ability to navigate a Windows environment
- Time management, organizational, research, analytical, and documentation skills.
- Ability to work independently, efficiently, perform multi-tasking, prioritize tasks, and meet deadlines.
Additional Qualifications:
- Outstanding knowledge of effective lifestyle behavior change such as tobacco cessation and weight management, and the role lifestyle plays in conditions such as diabetes and heart disease
- Demonstrated experience applying health behavior change theories and principles, such as the Transtheoretical Model and Motivational Interviewing
PAY RANGE: $55,000 - $65,000 per hour
