Reporting to the Performance Improvement Director, the Performance Improvement Specialist assists with the coordination, development, implementation, and evaluation of Independent Living Systems California (ILS-CA) Performance Improvement Program

The Performance Improvement Specialist may also assist with care management activities, as needed.

Position Responsibilities

  • Develop and implement audit and tracking tools, project planning forms, and data collection documents
  • Conduct ongoing performance improvement audits
  • Contribute to various performance improvement initiatives
  • Gather, retrieve, analyze, interpret and display data collected through audits, performance improvement initiatives and other projects
  • Identifying performance improvement opportunities
  • Prepare and present routine reports
  • Identify trends and present recommendations to improve individual and organizational performance
  • Provide consultative services to ILS staff regarding standards of practice, performance improvement and organizational compliance
  • Contribute to the development of performance improvement measures and outcome measures
  • Contribute to staff continuing education
  • Contribute to the implementation of the performance improvement program, meeting  the needs of consumers, staff and collaborators
  • Provide direct observation of ILS-CAre Teams to support continuing performance improvement
  • Conduct field visits to satellite offices, as requested
  • Assist with various collaborative projects
  • As requested, provide high quality care management and LTSS care coordination to Health Plan Members in a timely, effective and professional manner.
  • Complete other duties as assigned
  • Maintain active license and/or related certifications

Position Qualifications

Education:  vocational nurse (LVN), registered nurse (RN) or Social Work degree.

Licensure/Certification: Current California LVN or RN license or BSW or MSW diploma or transcript.

Minimum Experience:   One or more years of experience in managed healthcare and/or clinical case management.

Knowledge:  A thorough knowledge of performance improvement standards and techniques, as well as the ILS-CA|KP care management model. Competency utilizing ILS’ care management technological platform and the Microsoft Office platform.  Familiarity with the California Coordinated Care Initiative (CCI) and managed care environment (preferred). 

Physical Demands

This position may require duties including lifting and carrying up to 40 pounds and sitting for prolonged periods of time.  Good visual and auditory acuity as well as good manual dexterity and the ability to be readily understood are essential.  Must be able to work in a stressful environment.
