Peer Coach at Boulder 


A Boulder Care Peer Coach (PC) is a person who uses his or her lived experience of recovery from mental illness and/or substance use combined with skills learned in formal training to deliver peer support services with a focus on improving overall health and well-being, and quality of life.


Your Role as a Peer Coach


As a PC, you will report directly to our Peer Coaching Program Manager. In this role, you will help scale a fast-growing, high-impact health care organization while working directly with patients to empower them on their recovery journeys.

This position is unique in the flexibility and convenience that it offers. Ideally, you are located in Eugene, OR or Portland, OR, but you will have the opportunity to work from your own home and maintain flexible hours. You will occasionally be expected to join Clinical Team and Peer Coaching Team calls as well as provide coverage on weekends.

In this high-impact, patient-facing role, you will deliver compassionate, empathetic telephonic, text-based, and video-based (telehealth) patient engagement and monitoring, assisted by a clinical decision-support tool. You will also coordinate with multidisciplinary care teams via telehealth tools to guide treatment planning and delivery.


Your Responsibilities


At Boulder, you’ll be a part of a dynamic, fast-growing team, working to:

  • Assist patients in identifying barriers to achieving sustained recovery and support by empowering them in the recovery management process
  • Assess patient progress, functioning, and recovery stage in collaboration with patients and Care Team members, identifying short/long-term and whole-health goals, and treatment management plans
  • Educate patients about substance use, available support services, treatments, and the Boulder Care program components
  • Link patients to appropriate services and resources (e.g., housing, transportation, food) to reduce the likelihood of reuse and promote improved health and well being
  • Assist with managing patient appointments with Care Team members and referrals outside of Boulder Care
  • Maintain clear and proactive follow-up communication with patients and providers, delivering patient outcomes reports to Care Team members
  • Report changes in patient status (e.g., high risk for returning to use) and potential support services leveraged to other Care Team members in a timely manner
  • Facilitate placement for higher levels of care as needed while maintaining contact with patients to enable continuity and streamline care transitions


Experiences and Attributes


You bring to the table a demonstrated experience in peer recovery and support services. You’ve likely spent least 24 months in active recovery. Or, you may have experience, personally or as an ally, with the following: mental health, substance use disorders, substance use disorders while pregnant, Hep C (and/or HIV), PTSD, trauma LGBTQ, comorbid psychiatric, or criminal justice.


In addition, you have the following experiences:

  • Navigating adjunctive opioid use disorder (OUD) treatment resources and referral processes
  • Reaching out to local and regional resources or recovery centers and helping individuals leverage those resources
  • Handling multiple tasks, prioritizing, and problem solving based on severity of need with minimal supervision
  • Delivering clear written and verbal communication as well as extraordinary customer service and record keeping
  • Leveraging Google Suite (GMail, GoogleDrive, GoogleDocs, etc.) or MS Office, video conferencing and messaging services like Slack for communication and collaboration
  • Membership or certification with a relevant accredited body (CRM, PRC, or NCPRSS-National Certified for Peer Recovery Support Services) is preferred

  • A high degree of trustworthiness, integrity and professionalism and a relentless focus on patient confidentiality through compliance with HIPAA, 42 CFR Part II, the Boulder Care Code of Conduct and Code of Ethics, Boulder Care Standard Operating Procedures
  • Exceptional listening skills, cultural competence, and strong boundary management
  • Exceptional teamwork and a willingness to collaborate with/learn from interdisciplinary Care Team members including physicians, nurse practitioners, registered nurses, and care advocates in all aspects of care provision


We are looking for full time and part time candidates in all locations.

If hired, and you are working remotely, you must meet Boulder’s Remote Worker Requirements & Expectations which include items such as a quiet designated office space with a locked door and access to high speed internet.
